Project Manager and EX Footage

When creating a trimmed project in Premiere CS5, the resultant MXF files have blocky banding in the first few frames, and when attempting to open the trimmed project, Premiere shuts down with the "ippmpegdecoder.dll" fault.

Is this a known issue?
Is there a way to get Premiere to read these trimmed clips?
