Why HD didn't get to a fast start...

That's half the reason and the other is the fight over the format. Now that 1080p is finally becoming a standard, I think HD adoption will take off. With 1080p and blue-ray, there is enough quality improvement over upsampled DVDs to make the switch worthwhile.
