Blackmagic Intensity Pro Problem

I purchased a Blackmagic Intensity Pro in August for a new MacPro 2.8 Quad and I've been having a very strange problem. Sometimes it won't play back 1080i to my monitor via component. When this happens the video on the timeline on FCP 6 plays at double speed but the audio is fine. If I switch the output to a different setting I can see a picture but of course it's not 1080i.

I troubleshooted it with Blackmagic tech support who decided it must be a faulty board and had me return it. The tested it on a similar computer and found no problems at all.

Now I've reinstalled it and I get no video output at all on any setting. I called tech support and was told that they have heard of this problem only once or twice and they have no fix for it. So now I'm stuck with this card that doesn't work.

Has anyone else had this problem?

Here's my setup copied from the RMA email:

Detailed Problem Description:

I'm trying to view 1080i footage from the timeline. Sometimes it works fine. Other times there's no video to the monitor and the video on the timeline plays fast (like double speed).
