I spent a while searching old threads but to no avail for this seemingly common question. I have a local band who is willing to let me use their music. How do I write that up in contract form. Any guides to links or old threads would be very useful.

I did a Google on "music release contract" and a site came up first with sample contracts. One of them is:

You may have to tinker a bit or look for another one.
I've got one here in Spanish, no good huh?
