Matt box recommendations for w/a & standard lens?

I have a videocam with a 72mm lens. I also have a Canon screw on wide angle adapter which has a 109mm front I believe

Who makes a matte box with filter holders that will fit both the stock lens and onto the screw on Canon WD H72 wide angle adapter without vignetting?
