store. To qualify fill out this form in its entirety and include a copy of your sales receipt. Your sales
receipt must be dated between 2/1/14 and 2/28/14. Mailing must be postmarked by 3/31/14.

Shotguns:
Purchase the SGM-1X, SGM-2X, SGM-1000 or SGM-PII between 2/1/14 and 2/28/14 and receive free, a heavy duty
3-pin XLR male to XLR female balanced microphone cable. Cable length is 3’.
Mixers:
* Purchase any Azden FMX Series Mixer listed above between 2/1/14 and 2/28/14 and receive free, a full-size high quality monitor headphone supplied by a major name brand manufacturer. Headphones feature powerful 40mm drivers and swivel earcups, with around-ear leatherette cushions for long-lasting comfort. Frequency response is rated at 12Hz – 22KHz, cable length is over 8’ and terminates with a gold plated 3.5mm plug. A ¼” adaptor plug is provided.
