I have been away this week so haven't been able to sort this out.

But first - we recorded line in
Second - the sound recordists knew what they were doing - levels were set correctly.

If trim has no bearing on Line in - then it is not the problem.

I realise all is speculation until I get to proper test with a mixer.

Well let us know what you find out. Sounds like operator error to all of us , but maybe we'll all learn something.

Were the recordists monitoring sound from the camera?

Yes the recordists were monitoring sound from camera. Both picked up problem on first shot of the day. Levels were set with tone.
