Release form
 

Looking for all around release form. I'm shooting a documentary that follows a US chemistry professor around in Tanzania. He is trying to establish a manufacturing facilty for AIDS drugs. He will be lecturing at the school of pharmacy in Kilimanjaro but I'll also be interviewing some heads of state and I'm looking for a release form or form(s) that might fit the bill. I could have sworn I saw a good one on here a few years ago but I can't find it.

I've also done some web searches but I haven't found anything too promising. Any ideas out there? Leaving tomorrow but I'll probably have access to web. Probably.
