Ball Head recommendation
 
Can anyone recommend a ball head that will attach to a shoe bracket - I've a few cheapies that never stay tight so I'm looking for something that's engineered better.

The FLM CB18 from Speedgraphic in the UK is small but extremely strong:

I have half a dozen of them.

If you need some way to mount them in cold shoes get some of these Kaavie Dual Nuts:

I often use the above combined with these sort of flashgun stands to place small cams like the TM900 and GoPro and audio recorders:

They are longer than the normal flash stands so are more stable.

I have to add my vote for the Manfrotto 492LCD. I have one and plan to add another to my 'kit'. The Speedgraphic also looks like a sturdy ball-head, I just have never used one.

also keep in mind there are cheap adapters to go from 3/8 to shoe and/or to 1/4 then to shoe, which opens up any regular ballheads. i've gotten to like the one that comes with this kit quite a bit. huge capacity, nice size ball.
