Cavision matte box

Hi guys

I'm asking this over on hbs with regard to steadicam but I know Tim owns this setup so I thought I'd ask here also, hope thats ok. Anyway, Cavision 16:9 mattebox for hd100 arrived today. In the spec it says it comes with the plate for jvc cameras but I can't for the life of me see how this is going to attatch, there are no screws for starters. What am I missing thats going to make this work?

Mine didn't come with the screws either, but luckily I was able to use the screws from the JVC V-plate I had already purchased with the quick-release.
I think Cavision assumed you have the quick release already.

I don't have either of those. Only been using steadicam up till now. I guess I'm off hunting again.
