Release form for extras

Hello everyone,

I got hired to cover a band playing during a prom night and they have asked me to capture shots of the kids dancing and showing a good time. The school is a bit reluctant in doing so as they do not want to be liable if the video would make it on Youtube or for commercial use i.e. TV. The Band will only use this video in their studio to show potential customers.

I'm looking for an actor or extra release legal form that i could get some of the willing students to sign to basically cover our and the schools buts.

Do any of you have such a document and would you be willing to share it?
