Music contract/license/release for music use in film at DVinfo.net

Hello,

I make movies, a friend-composer supplies the music. We would like a simple contract that allows use of his music for the life of the film with the usual aspects: publicity, use in cd compilation of film music, etc.

Know where I can find one?
