log sheets

anyone got example log sheets? what things are important to note apart from time in, time out, length, video quality, audio quality, comments?

What jumps to mind:
length may be unneccesary; can be figured from in/out times.

I always include "Number of takes".

I find quality, whether video or audio, is frequently best determined in post. But that depends on what equipment you're using while recording.
